<p><strong>Step 2: </strong>Obtain a set of your fingerprints.</p>
<ul>
<li>Provide the original fingerprint card. Previously processed cards or copies will not be accepted.</li>
<li>Your name and date of birth <strong><em>must be</em></strong> provided on the fingerprint card. Fingerprints should be placed on a <a href="http://www.fbi.gov/about-us/cjis/background-checks/standard-fingerprint-form-fd-258">standard fingerprint form (FD-258)</a> <strong> </strong>commonly used for applicant or law enforcement purposes.</li>
<li>Include rolled impressions of all 10 fingerprints and impressions of all 10 fingerprints taken simultaneously (these are sometimes referred to as plain or flat impressions).</li>
<li>If possible, have your fingerprints taken by a fingerprinting technician. This service may be available at a law enforcement agency.</li>
<li>To ensure the most legible prints possible, refer to the <a href="http://www.fbi.gov/about-us/cjis/fingerprints_biometrics/recording-legible-fingerprints">Recording Legible Fingerprints brochure</a>.</li>
</ul>
<p><strong>Step 3: </strong>Submit payment.</p>
<ul>
<li>Option 1: Obtain a money order or cashier’s check for $18 U.S. dollars made payable to the Treasury of the United States. Please be sure to sign where required.</li>
<li>Option 2: Pay by credit card using the <a href="http://www.fbi.gov/about-us/cjis/background-checks/credit-card-payment-form">Credit Card Payment Form</a>.<strong><em> </em>Don’t forget to include the expiration date of the credit card that you are using.</strong></li>
<li><strong>Important note: </strong>Cash, personal checks, or business checks WILL NOT be accepted.</li>
<li>Payment must be for the exact amount.</li>
<li>If the request is for a couple, family, etc., include $18 for each person.</li>
<li>If the request is for multiple copies per person, include $18 for each copy requested.</li>
</ul>
<p><strong>Step 4: </strong>Review the <a href="http://www.fbi.gov/about-us/cjis/background-checks/fbi-identification-record-request-checklist">FBI Identification Record Request Checklist</a> to ensure that you have included everything needed to process your request.</p>
<p><strong>Step 5: </strong>Mail the required items listed above—signed applicant information form, fingerprint card, and payment of $18 U.S. dollars for each person or copy requested—to the following address:</p>

			<content:encoded><![CDATA[<p>Access to criminal history summary records maintained by the DOJ is restricted by law to legitimate law enforcement purposes and authorized applicant agencies. However, individuals have the right to request a copy of their own criminal history record from the Department to review for accuracy and completeness. Requests from third parties are not authorized and will not be processed.</p>
<p>To receive a copy of <strong>YOUR</strong> criminal history record, individuals must submit fingerprint images, pay a $25 processing fee to the DOJ, and follow the instructions below.</p>
<h2>CALIFORNIA RESIDENTS</h2>

<li>Enter “Record Review” on the “Reason for Application” line.</li>

<li>Take the completed form to any Live Scan site for fingerprinting services.</li>

<p>Fingerprinting services are available at most local police departments, sheriff&#8217;s offices or any public applicant Live Scan site. <h2>OUT OF STATE RESIDENTS:</h2>
<h3>Applicants Living Outside California/United States must submit manual fingerprint cards (Form FD258).</h3>
<p>Print out the Application to Obtain Copy of State Summary Criminal History Record (<a href="http://oag.ca.gov/sites/all/files/pdfs/fingerprints/forms/BCIA_8705.pdf?">Form BCIA 8705, pdf</a>) and follow the instructions on the form. Please contact your local law enforcement agency for fingerprinting services. PLEASE NOTE: Your fingerprint card must contain your full name, date of birth, sex and return mailing address. If you are having difficulty in obtaining a blank fingerprint card, please contact the Record Review Unit at (916) 227-3849.</p>
<p>Manual submissions must be accompanied by either personal check drawn on a U.S. bank, money order or certified check. The check or money order must be made payable to the “California Department of Justice.” Mail your application, fingerprint card and processing fee to:</p>

<p><strong>Once you have received your Record Review response,</strong> if you want to challenge the contents of your criminal record, you must complete the &#8220;Claim of Alleged Inaccuracy or Incompleteness&#8221; (form BCIA 8706), which will be included with your Record Review response if there is criminal information on your record. Mail the completed form, along with a copy of your criminal history record, to the address indicated on the form. Your challenge must specifically state the basis for the claim of inaccuracy or incompleteness and include any available proof or corroboration to substantiate your claim.</p>

			<content:encoded><![CDATA[<p>Most live scan applicants are required to submit some type of identification to the Live Scan technician, we have provided information below that may help you determine valid forms of identification.</p>
<p><strong><br />
Prior to rolling fingerprint impressions, it is imperative that the identity of the applicant be established.</strong> It is the responsibility of the fingerprint roller to check the identification of individuals being fingerprinted. A valid photographic identification must be presented by the applicant and checked closely by the certified fingerprint roller taking the fingerprint impressions. This ensures that the applicant’s identity is validated against proper identification which results in accurate information submitted to the DOJ.</p>
<p>The DOJ requires certified fingerprint rollers <em>accept only current, valid, and unexpired picture identification documents as a primary valid identification</em>. As such, a certified fingerprint roller must examine the photo identification presented to ensure all of the following:<br />
•<br />
The personal descriptor information (name, gender, date of birth, etc.) matches the information provided on the “Request for Live Scan Service” form (BCII 8016);<br />
•<br />
The expiration date has not elapsed; and,<br />
•<br />
The photograph reasonably resembles the Applicant’s physical appearance.<br />
Acceptable Primary forms of photo identification include any of the following:<br />
•<br />
California Driver’s License<br />
•<br />
Department of Motor Vehicles Identification Card<br />
•<br />
Out-of-State Driver’s License<br />
However, in the absence of a primary form of identification, a certified fingerprint roller may accept one or more of the following as Secondary forms of Identification, but only with two of the supplemental documents noted on the next page:<br />
